HISTORY of PRESENT ILLNESS

Review of Systems (positives and pertinent negatives noted.): He walks approximately 1 mile in 30 minutes without dyspnea or chest discomfort. No blurry vision, no change in appetite. He has gained 20 pounds over the past year because he reports being "stressed" at work.

Other Medical History:
Hypertension
- Diagnosed 5 years ago and treated with ACE inhibitors. No side effects noted from ACE and BP control has been excellent on the medications.

Past Surgical History: None

Family History:
Father had a non-fatal MI at age 53 and a fatal MI at age 55. Mother died at age 71 in an accident. He has two living brothers, one of whom has CHD.

Allergies: None

General Health Maintenance: Last tetanus shot 4 years ago. PSA normal with a normal prostate examination 6 months ago. Colonoscopy 4 years ago, no polyps or tumors found.
